However his accuracy was a little off and even after repeating the solutions, it seemed that he could not replicate it.

However a reoccuring weakness is that he still doesn't understand the fundemental reasoning when multiplying or dividing with algebraic expressions. I'm more than happy to go over it with him as many times as needed but it is concerning to need to repeat it so many times. I'm happy to rework the concept with him but he must ensure to do the homework I set.

I would want him to revisit simple principles of algebraic expressions, along with the terminology that's associated with it (e.g. coefficient, like terms, different terms...).
